Please Note: This certificate entitles the recipient to one phase of orthodontic treatment and includes on set of retainers. The term “orthodontic treatment” includes early treatment, full braces, Invisalign treatment and any other required orthodontic treatment in a LOA office. There is no age limit. This certificate cannot be applied towards previous treatment, in-progress treatment, and can only be redeemed at LOA Orthodontics. EXPIRES November 1, 2024.
