Optometry, Eye Exam, Contact Supply

This certificate will entitle you to an eye exam, a contact lens fitting, and a 6-month supply of contacts with Dr. David O'Leary's office (Birmingham, AL).  Contact information will be given to the winner to arrange the visit.
